What is Complex PTSD Therapy?
Complex PTSD (CPTSD) is a diagnosis recognised in the ICD-11 that develops following prolonged or repeated exposure to traumatic events — particularly when escape is difficult or impossible. It is associated with childhood abuse and neglect, domestic violence, human trafficking, prolonged wartime exposure, or being held in conditions of captivity. Unlike standard PTSD, CPTSD includes three additional feature clusters beyond the core PTSD symptoms: difficulties with emotional regulation, persistent negative beliefs about self (often involving deep shame), and difficulties with relationships and connecting with others. Complex PTSD therapy at Mindful Talk Therapy Scotland follows a phase-based approach: Phase 1 focuses on Safety and Stabilisation — building the therapeutic relationship, developing emotional regulation skills, and creating a foundation of safety before any trauma processing begins. Phase 2 addresses Trauma Processing — working with traumatic memories at a pace you control. Phase 3 focuses on Integration and Reconnection — developing a new relationship with yourself and the world, and building a future that isn't defined by what happened. Treatment draws on TF-CBT, Schema Therapy, compassion-focused techniques, elements as appropriate.

This is not a failure of the treatment — it is the correct and safe approach. Progress is gradual but meaningful.
